This form is your opportunity to rate the level of participation of your team teaching members. Use the guidelines below to rate the performance of each group member (including yourself).

Print and fill out the form. Obviously, you need to be honest in evaluating yourself and others for your team-teaching project.  This is due, in a sealed envelope, by the last day of class.

  B-   Usually did what he/she was supposed to do, acceptably 
 prepared and cooperative; helped make team teaching 
 better. 
 C+  Often did what he/she was supposed to do, but minimally
 prepared and cooperative; other people took the lead.
 C   Sometimes failed to show up or complete assignments; 
 sometimes prepared and sometimes not.  
 C-  Often failed to show up or complete assignments, rarely
 prepared. The group had to carry this person’s share of 
 the work.
  D  Consistently failed to show up or complete assignments,
 unprepared. Others had to carry this person’s share of the
 work.
  D-   Practically no participation.
    No participation at all or actively disrupted the group
